A parent of a consumer under 21
ELIGIBILITY
Because this is a state-funded home health program, there are a few eligibility requirements.
The consumer (person needing care) is actively enrolled in ALTCS
The consumer (person needing care) is a AZ resident & 18+
The consumer (person needing care) needs assistance with daily activities
The consumer meets need requirements including help with: bathing, grooming, dressing, etc.
